Referrals Made Easy
Thank you for referring your patient to The Dallas Heart Group. The physicians at The Dallas Heart Group value the relationships we have with our referring physicians and welcome any feedback or comments you may have regarding our practice. We take care to reinforce your role as your patient's primary care physician and our role as a cardiac consultant.
Clear and appropriate communication with our referring physicians is important to us. If desired, our staff will call you to confirm your patient has made an appointment with one of our physicians. After the patient's visit, our physician will forward a summary of diagnostic reports and treatments to you as soon as it becomes available.
For your convenience you can either call us or complete our Referral Form and fax it to our office. Please do not submit patient information via e-mail as it is not secure.

Our Referring Physicians Service is available Monday through Friday from 8:30 a.m. to 4:30 p.m. at 972-566-7733, then press 2. You can request to speak to a physician directly or call one of the numbers listed below to speak with one of the medical secretaries.
