Planning Your First Visit — If you want to schedule an appointment, please call 972-566-7733, then press 2. You can complete your patient forms prior to your first visit by downloading the New Patient Packet. Please bring your completed form, insurance cards, a list of current medications and copies of relevant medical records to your appointment.
